Sick notes requests
- If a new sick note (Med3 fit notes) is required and it covers a period of less than 7 days then you do not need to see a doctor. You can fill out the self certification sick note and hand it directly to your employer. The self certified sick note (SC2 Form) can be accessed by clicking here.
- If a new sick note is required and it covers a period of more than 7 days then you will need to make a routine appointment with a doctor. If there has been a period between the end of your last sick note and a new request or a sick note with the medical reason changed, then the request counts as a new sick note.
