Changing your details in the surgery will update your contact information throughout the NHS unless you have specifically opted out of linking your records with the NHS network.

• To update your telephone number – contact the reception by phone, coming into the surgery or fill out the form below.
• To update your address (and for your family) – the change of address form will need to be filled out at the surgery or fill out the form below.
• To change your name and/or date of birth, you will need to bring a form of photo ID (such as passport or driving licence) to prove the new update is accurate.
